CDC Guidelines for Covid-19 are being followed in our Health Offices and for Mandated School Screenings to ensure your child’s safety.

 PA School Health and Screenings

 PHYSICAL FORM is required for:   

• Upon New Entry to any grade
• Kindergarten
• 6^thgrade
• 11^th grade

 *The physical exam can be from an appointment up to one full calendar year prior to the start of school for the required year.

 DENTAL EXAM FORM is required for:

• Upon New Entry to any grade
• Kindergarten
• 3^rd grade
• 7^thgrade

 *The Private Dental exam can be from an appointment after January 1^st of the year prior to the required year.

* Please put STUDENT NAME, BIRTHDATE and SCHOOL BUILDING on any Health or Dental Forms sent by fax or email so your child’s health record can be updated.
